Dr. Lewis is a United States Air Force veteran who flew 168 combat missions and served as a senior flight surgeon for the Arizona Air National Guard at the rank of lieutenant colonel. Since retiring from the armed forces, Dr. Lewis has focused on providing comprehensive care to people living and working in his community. After completing his undergraduate degree at Oklahoma State University in Stillwater, Oklahoma, Dr. Lewis earned his medical degree from the University of Oklahoma (OU) College of Medicine in Oklahoma City. Kristin Olson, DNP, FNP-C

Kristin Olson, DNP, FNP-C, is a board-certified family nurse practitioner at Umbrella HealthCare in Phoenix, Arizona. Practicing in healthcare for over 10 years, Dr. Olson received her Bachelor of Science in Nursing (BSN) from the University of Nevada, Las Vegas. Her extensive nursing background includes specialized work in cardiology, pre-operative/post-anesthesia care, and high-risk obstetrics. She is also a trained Forensic Nurse Examiner (FNE). After spending several years as a nurse, Dr. Olson returned to school and completed her Doctor of Nursing Practice degree from the University of Arizona. Shortly after, she became board-certified as a Family Nurse Practitioner. Dr. Olson currently works in internal medicine, conducting inpatient management on post-acute rehabilitation and long-term acute care patients. She also works in a busy pediatric clinic and has a third role in hospice and palliative care. Meta Galabe, DNP, PMHNP-BC

Psychiatric Mental Health Nurse Practitioner Meta Galabe, DNP, PMHNP-BC began her career as an RN working with patients in Phoenix, Arizona. Her experience includes working with patients in a psychiatric residential treatment facility, hospice, and general psychiatric medicine. Nicole Larsen,FNP-BC

Nicole Larsen, FNP-BC, is an experienced family nurse practitioner who provides high-quality services as part of the Umbrella HealthCare team in Phoenix, Arizona. She is committed to promoting improved health and wellness through compassionate, patient-centered medical care. Nicole attended Central Arizona College in Coolidge, Arizona, for her Associate of Science in Nursing degree, then earned her Bachelor of Science in Nursing degree at Grand Canyon University in Phoenix, Arizona. She is an active member of the American Association of Nurse Practitioners. Nicole returned to Grand Canyon University to complete her master’s degree in nursing. In her previous roles, Nicole has been a case manager and clinical supervisor, providing adult and pediatric care to patients in their homes. She has undergone in-depth training and has extensive experience in many aspects of specialized medicine and primary care. These include hypertension (high blood pressure) management, diabetes care, addiction medicine, ventilation management, developmental disabilities, neuropathic pain, and caring for patients with multiple acute and chronic illnesses. Nursing background: Neurological Stroke Unit, Telemetry Cardiac Unit, and Emergency Room/ Trauma Nursing.

Theresa J Fryer, FNP-BC

Highly skilled family nurse practitioner Theresa Fryer (known as Jill), FNP-BC, provides exceptional primary care services to people in and around Phoenix, Arizona, as part of the Umbrella HealthCare team. Jill is especially devoted to geriatric and palliative (end-of-life) care and has a passion for helping patients and their families through complex medical journeys. Jill grew up in Spring, Texas, near Houston. She majored in biology at the University of Texas at Austin in Austin, Texas, and after graduating, started work as a project coordinator at a treatment therapy center for children with autism. She realized her true passion was providing direct care to patients, so she went back to school to study for a nursing degree. Jill received her bachelor’s degree in nursing from Texas Woman’s University in Houston, Texas, and began her nursing career at the University of Texas MD Anderson Cancer Center in Houston. After five years of experience in cardiology and oncology, Jill went back to her nursing education. She earned her master’s degree specializing in family practice from the University of North Florida in Jacksonville, Florida. Throughout her nursing career, Jill has gained invaluable experience across a variety of settings, including hospital inpatient care, outpatient care, and home care. She has spent more than 13 years expanding her knowledge and now brings that expertise to her role as a primary care provider. When she’s not at Umbrella HealthCare, Jill enjoys spending time with her husband, their two children, and the family’s two dogs. They often head outdoors in the beautiful Arizona sunshine, enjoying hiking, camping, and swimming.

John P Crockett, LAC

John “JP” has been a therapist for the last 3 years. In which time he has worked at Lewis prison, Honor Health Hospital and currently at Umbrella Health where he works with individuals/ Couples and those who are in group homes. He counseling is based off his favorite therapist Milton Erickson. “It is really amazing what people can do. Only they don’t know what they can do.”
